Head of Service Integrated Discharge Teams East Kent, East Kent Hospitals NHS Foundation Trust
Claire Casarotto, BSc(Hons), PGCert(d), is a physiotherapist with a background in acute medicine and specialist experience working within Emergency and Urgent care. She had extensive clinical experience in acute and community environments before moving into an organisation-wide role in leading discharge facilitation and coordination in 2009. She has a Post-graduate certificate with distinction in Management in Health and Social Care.
Claire formed part of the development team for the Integrated Discharge Teams in East Kent, subsequently leading the team at Kent & Canterbury Hospital for 18 months to deliver a level of excellence in integrated discharge facilitation to which the other areas in East Kent could aspire. She became the Head of Service for IDT in 2016, and provides operational and strategic leadership for both East Kent Hospitals NHS Foundation Trust and Kent Community Health NHS Foundation Trust in further development of Integrated Discharge planning alongside Lisa Martin, Short Term Pathways Team Manager, Kent County Council. 
She led the operational integration from health perspective of the Hospital Optimisation process and was influential in the development of the East Kent model of Discharge to Assess in 2015 and Home First Programme in 2016.
Most recently the East Kent IDT has been recognised by NHS England as an example of excellence and will be featured as a case study in the new NHSE Quick Guide - Integrated Discharge Teams.
Claire continues to be invited to present her knowledge and experience in Hospital Discharge planning at a national level alongside Lisa Martin.
