Clear and accurate data on patients and individuals

What does data mean in the healthcare space and should we be considering the benefits of a centralised data source? Does this mean a decentralised platform where patients own all their data and grant access - similar to crypto keys but not blockchain or is this a single countrywide Electronic system controlled by the state. What does good and bad data look like and where does the data come from.
