Optimising patient care using digital health solutions

Hywel Dda Health Board has outlined a “digital response” to support its strategic vision of working together to drive excellence in care for its patients and communities, which includes Technology Enabled Care (TEC) and telehealth. TEC is recognised internationally as integral to improving the wellbeing and independence of individuals in the community who have care and support needs. Telehealth is also well documented in relation to prevention and pro-active management of individuals at risk of, or living with, chronic conditions, supporting them to self-manage their conditions, remain more independent, reduce hospital stays, allow early hospital discharge, reduce dependency on primary care, and support rehabilitation services. This session will follow how Hywel Dda has introduced new ways of working and the opportunities telehealth presents to transform patient experience.
