National service model for an integrated community stroke service

Rebecca Fisher will provide an overview of the Integrated Community Stroke Service (ICSS) model and the National Rehabilitation Pilot project. Simone Browning is the programme lead for the North Central London National Rehabilitation Pilot. Simone will describe a pan-sector resource providing the key components of the ICSS (vocational rehabilitation, psychology, social work, access to specialist therapy and dedicated nurse input) achieved through a collaboration across five community teams and hospital-based stroke teams.
