Transforming CVD Prevention: the UCLPartners Proactive Care Frameworks

The UCLPartners Proactive Care Frameworks are a major innovation to help practices and PCNs transform management of long-term conditions as we emerge from the pandemic. The frameworks are now part of Proactive Care @home, a national approach from NHS England and NHS Improvement  that has been taken up in a third of integrated care systems. Frameworks have been developed for atrial fibrillation, hypertension, hyperlipidaemia and type 2 diabetes as well as COPD and asthma. The frameworks and resources are free. They include:

·       Comprehensive search tools built for EMIS and SystmOne that risk stratify patients so that clinicians can prioritise in their work – ensuring that everyone is offered appropriate care but that those at greatest risk are seen earliest, optimising treatment and helping to meet QOF and other targets

·       A broad range of protocols, training guides and digital resources to support primary care staff such as healthcare assistants to deliver structured support for patient education and self-management.
