Early intervention in social care: a tech-enabled approach

Actionable data insights can directly impact the cost and quality of healthcare delivery in social care. Technology, in combination with partnerships between social care prescribers, providers and health systems, make up important ingredients in achieving better patient outcomes and reducing costs. Over the last 18 months, Birdie has partnered with key stakeholders across care pathways to enable care workers to track and report early signs of deterioration in care recipients’ health and well being at home. This information is then shared immediately with clinicians for monitoring and timely intervention. This session will focus on the key considerations and impact of this integrated care delivery model, and the role of technology and human-centred approach in providing timely treatment.
