Citizen Lead, Professional Record Standards Body (PRSB)
Sam Bergin Goncalves is a parent carer of a young adult with complex needs. Sam has been using technology and a Multi Media Advocacy approach to support her son Shane’s Education, Health and Care planning for the past 8 years. The use of technology has transformed how those supporting Shane deliver their services seeing Shane as a person and not just his disability resulting in a more integrated approach to health and care. Focussing on Aspirations has been paramount in this approach which was the main driver for Sam to set up a personal bespoke adult service for her son. Sam regularly delivers person centred planning training and has presented at a number of National events on the importance of information sharing and individual’s wellbeing. Sam has held a number of voluntary posts with organisations supporting people with learning disabilities. Sam is currently the citizen lead for the PRSB representing the interests of the public in the development of record standards to accelerate improvements in social care.
